How to apply
First, you need to confirm you’re eligible to receive funding. You can click here to read the full eligibility checklist. Essentially, these are the requirements:
- You’re aged between 7 and 65;
- You’re an Australian citizen or permanent resident, living in Australia; and
- You have a disability caused by a permanent impairment, and you wish to access funding to receive support.
Then, if you think you’re eligible, apply for an NDIS plan either by giving the NDIA a call on 1800 800 110, or by completing an Access Request Form. You can download this form, or call or email the NDIA to get your copy.
Once you receive your form, fill out all the information in Section 1, and sign and date it. Then get your treating healthcare professional to complete Section 2.
You also need to provide evidence to show your eligibility for NDIS funding. This covers things like:
- Personal details including age, citizenship, and disability.
- Supporting evidence that tells the NDIA more about your disability. This can be supplied by your regular healthcare support practitioners, such as your GP, physiotherapist, or speech pathologist. You can read more about this here.
Once you submit your application, the NDIA then assesses your eligibility for an NDIS plan. This typically takes around 21 days.
Receiving your NDIS plan
Once you receive notification that you’re eligible to receive NDIS funding, the NDIA will be in touch to organise a planning meeting. It can be either over the phone or in person—whatever suits you best.
In this meeting you’ll discuss the goals, tasks, or activities you’re working towards, so come to your meeting prepared with a list of your current supports, and if possible, an idea of the supports you’ll need in the future.
Your NDIA planner will assess your information, discuss your plan with you, and advise on how long it may take to receive your NDIS plan.
The NDIA then gets to work to create your NDIS plan. Once it’s ready the NDIA will send it to you directly in the format that’s easiest for you to use.

How to use your plan
Once you receive your NDIS plan, you’re ready to connect with the NDIS service providers in WA that will help you to achieve your goals.
If you need help making the most of your plan, your NDIS contact can hold an implementation meeting with you. In this meeting, they can discuss with you how to find the right NDIS service providers for you, and how to use your NDIS funding.
The NDIS will contact you before the end date of your plan for a check-in to talk to you about your current situation, check on your wellbeing and make sure your NDIS supports are meeting your needs. They will also talk to you about booking in your plan reassessment meeting, where you will discuss whether any changes are needed to your NDIS plan.
